Does California Medi-Cal cover nursing-home care?

The short answer

California Medi-Cal can cover medically necessary long-term care in a skilled nursing facility for an eligible member. DHCS says institutional long-term-care services are generally arranged through the member's Medi-Cal managed care plan. Financial eligibility, medical need, and the way care is authorized are distinct questions. Ask the plan and county which step applies to your family member.

Medi-Cal is California's Medicaid program. DHCS is the California Department of Health Care Services.

What care does DHCS describe?

DHCS says long-term-care services in skilled nursing facilities are part of Medi-Cal managed care statewide. Its member guide also lists other institutional settings. The type of facility and the member's care needs determine which service rules are relevant.

Who arranges the care?

DHCS says the Medi-Cal managed care plan works with doctors, hospitals, and providers, and coordinates medically necessary services. Some services may still be delivered outside the plan. If the person has Medicare too, ask the plan how the benefits work together for the proposed stay.

How is this different from qualifying for Medi-Cal?

Coverage of a service does not mean a particular person is eligible or that a particular stay has been approved. California's asset test applies to some groups, including nursing-home residents, and the income rules are separate. A spouse living at home may have added protections.

What is the next useful call?

If the person already has Medi-Cal, contact the managed care plan about the proposed facility and medical authorization. If the question is whether the person qualifies for Medi-Cal, contact the county Medi-Cal office. DHCS lists ombudsman and helpline routes for member questions and problems.
